Bravado's review obviously deserves a thorough response.  We were disappointed in the response because all communications from Bravado had been professional and to the point and did not reference any problems.  Some specifics:
1.  We were clear about travel schedules and how they would impact communications.
2.  We did more then just provide an outline.  We provided sources, a bibliography, DVD lists and even  offered to buy them and send them if their local library did not have them available.
3.  We did not set a length requirement, but we did set a "substance" requirement and gave examples of what we did not want.
4.  The first draft was not acceptable from a substance perspective and had many typos and mistakes.  We asked for it to be redone.
5.  Bravado obviously did work hard to re-write the first draft and the next submission did have more substance.  However, it still did not meet our requirements for publication and still had typos and mis-used words.  We considered whether it would be easier to finish the project with Bravado or to do it ourselves, or re-source the project.  We decided it was best for us to stop the project with Bravado.
5.  We had agreed on 25% at initiation, 25% after a successful draft and 50% after final draft.  In our view, we did not get to a successful first draft.  We paid Bravado the first two payments and at termination offered to pay half of the final payment which was accepted.
6.  Her draft will not be published.  We have hired another writer to complete the project.
