Description: What I already have: I wrote the eBook, and it is 63 pages long. It chronicles my journey and struggle to find permanent weight loss solutions, and solutions to several other health problems I had, which was finally corrected by getting my hormones in balance. It details the importance of hormone balance as a treatment and cure for weight loss, and for several different health conditions, and as a means to achieving 'perfect health'. The book covers the importance of finding a doctor who understand hormones and their relationships to overall health, and finding one who understands the need for accurate testing, and prescribing bio-identical hormones rather than synthetic chemicals or hormones derived from pregnant mare's urine.

The book also explores several causative factors for hormones getting out of balance, such as diet, ordinary household chemicals, sugar, cosmetics, etc, and the importance of proper nutrition and supplementation.

The target audience is anyone (male or female, although the primary audience is female) who is overweight, suffers from chronic fatigue, fibro-myalgia, stress, low libido, fuzzy-headedness, high cholesterol and triglycerides, and elevated blood sugar levels.

Description: We would like to outsource the layout of a construction company's anniversary magazine. Because of a question regarding how to position ads, we will need a dual quote -- one for all the ads being in the back (similar to a high school yearbook); and the other quote to for the ads included within the story content.

Thus, the first quote is for 68-72 pages of content and ads, mixed together.

The second quote is for a 40-page layout of content, with 28-32 pages of ads slotted after the content. Much less work.

Here are the details:

Perfect bound.  
Images to be set at 300 DPI, Binary.
Final output to be delivered in Quark 6.5. We will create PDFs here.

1.  We will FTP finished tex, images, and graphics. We expect some color correction and enhancement necessary with the images.

2.  Output to be in story sections, not signatures (per printer requirements).

3.  Would like two design concepts within 5 days of receiving the job; could be the cover and key pages of each to show decision makers what it would look like. If this is an issue, let me know.

4.  Please advise on your policy for revisions.

If you have any magazine layouts in your Web portfolio, please provide a link. Brochures and logos are nice, but we prefer to see how you approach a magazine.
